Violations: Cooked brown rice, was not cooled from 135°F to 70°F in 2 hours and/or from 135°F to 41°F within 6 hours, after preparation. Observed cooked brown rice 86°F product center left over night in the cooking equipment. Person in charge stated he forgot the product was left in cooker, product voluntarily discarded and denatured due to time temperature abuse. Observed plastic containers being used in daily food prep activities, which are scored cracked repaired with wire and not durable under this usage condition Plastic utensils used for cooking are deformed and misshapen from heat, and are not durable under normal use in the food facility. Observed air conditioner condensation waste water dispensed in the vegetable sink. Waste line was removed and the sink was cleaned and sanitized for compliance. Condensation water from the food facility air conditioning unit is not being disposed of properly. Owner installed a drain line into the vegetable prep sink. Corrected on site.

An open employee’s beverage container was observed in prep area, a food preparation area. Food employees observed in prep area, not wearing proper hair restraints, such as nets or hats. Observed containers of food stored directly on the floor in walk-in cooler area, rather than 6 inches off of the floor as required. Food Employee is not using available sanitizer test strips or test kit to determine sanitizer concentration.Can opener had old dried food residue on the blade observed pizza prep refrigerator, with an accumulation of old food residue, debris on non-food contact surfaces. Fan guards in refrigerator have build up of static dust. Observed no chemical sanitizer being used on equipment and utensils after washing and rinsing them, and prior to use. Reviewed proper cleaning procedure with Person in charge. Outdoor storage area for waste is located outside of facility and is creating a potential insect or rodent hazard for the food facility.
